Dear Colleague:

Thank you for your interest in the Wraparound Major Medical
Plan. Because of the favorable response, we have decided to
go ahead with the plan which will have an effective date of
June 1, 1983. All applications for the Wraparound Plan must
be received by the PSC no later than June 20, 1983. This
will be your only opportunity to join the plan until the
re-opening period of June, 1984.

If you have already filled out an application, you need only
send your check in the amount indicated below.

If you are a member of our present Blue Cross/HIP Plan,

you wi e automatically transferred into the new plan
effective June 1, 1983. If you do not wish the new plan,
please notify us at Once so that we can make arrangements ©
“to have you billed directly with no lapse of membership. We
are enclosing a PSC Membership Application for those of you
who have not yet joined. You know the Wraparound is only
available to PSC Members.
